Thank you for your interest in Your Clinic. Save time by printing and filling out the New Patient paperwork. If you are insured, please make sure to bring your current insurance card at the time of your appointment. If you do not yet have your card, please call your insurance company and make sure you have the following information before your scheduled visit:

  • Insurance Company,
  • Member ID,
  • Group Number,
  • Copay amount, if any.

If you are changing doctors and would like us to request your records, please fill out the Authorization for release of information form below and bring it with you to your scheduled appointment. Please bring the doctor's phone number and fax number if possible.

For individals and families that do not have health insurance, we offer affordable prices and flexible payment plans.


Commercial Insurance Plans

To better server our patients, we accept a wide variety of insurance plans. Please call your insurance company to verify that we are in network with your specific plan by dialing the phone number on the back of your card.
